There is no contract or obligation beyond the one month of care. The plan starts on the day of the first visit and will expire one day less than one calendar month later. For example, if a person initiates care on Thursday June 5th, their plan will expire on Friday July 4th.

Office hours are: Monday, Wednesday and Friday in Phoenix from 10-1:30 and 3-7; Tuesday and Thursday in Avondale from 10-1:30 and 3-7. We do not take appointments for existing patients. Walk-ins are welcome. We do ask that NEW patients call to schedule their first appointment ONLY.
